Outreach Carer

Here at Flourish Home Care the following Outreach positions have become available due to the increase in our growing outreach program.

We have 2 positions 9-3pm and 2 positions 9-5pm and this will include a Saturday once a month and a Sunday once a month.

Are you passionate about providing companionship, person centered care tailored to suit a service users wishes and needs?

Things will include-

  • Shopping

  • Appointments

  • Companionship

  • Light cleaning and cooking

  • Arranging post and bills

  • Engaging activities

  • Enjoyable days out

The list is endless.

Own vehicle and full driving license are required along with business insurance as you will be taking service users out.

Senior Community Carer

We are looking for a Senior Community Carer to cover the St Austell, Par & St Dennis areas and now reaching out to Truro and surrounding areas.

Role Overview:

As a Senior Carer, you will play a key role in supporting both service users and our dedicated care team. You’ll be responsible for overseeing care delivery, conducting work-related checks, supporting and mentoring care staff, and ensuring the highest standards of service are consistently met across all care calls.

Key Responsibilities:

• Carry out regular spot checks, work place assessment and medication observations to maintain quality standards

• Provide hands-on care, including personal care, medication support, and companionship, as part of scheduled care calls

• Maintain accurate records, reporting changes or concerns to management in a timely manner

• Communicate effectively with service users, families, and other professionals involved in care

• Promote a culture of dignity, respect, and empathy at all times

Requirements:

• Previous experience in a care or senior care role (essential)

• NVQ Level 2 in Health & Social Care (or working towards)

• Excellent communication and leadership skills

• Ability to work independently and as part of a team

• Full UK driving licence and access to a reliable vehicle (essential for community care)

Night-Time Live In Carers

We are looking for night time live-in carers (which will involve both sleeping and waking nights) The role will involve providing overnight care and support to an individual in their own home, ensuring their safety, comfort, and well-being throughout the night. This can include personal care, medication management, companionship, and assistance with household tasks. The specific duties will vary depending on the client's needs and the care plan.

Day-Time Live In Carers

A live-in care (days) job involves providing comprehensive support to an individual within their home, focusing on daily tasks and companionship. The carer's responsibilities include personal care, meal preparation, light housekeeping, medication management, and emotional support. They also assist with mobility, errands, and engaging in activities that promote the client's well-being and independence.

  • Every other weekend required.

Outreach Worker

We have an opportunity to work with two young men in St. Austell and the surrounding areas. They are both fun, energetic and friendly gentlemen that live at home with their families. They are both active and enjoy a range of interests such as walking, swimming, shopping and other activities.

Supporting these young men at home means you get to work on a one to one basis, really focusing on the person, their preferences, needs and outcomes for the day. The support you provide will be centred around each person, developing daily living skills, accessing a wider community, personal care and medication needs.

  • No challenging behaviour.

  • UK Driving licence required.

Temporary Deputy Manager

We are seeking a dedicated and proactive Deputy Manager to support the Registered Manager in the smooth day-to-day running of our service. This role is ideal for someone with strong leadership skills, excellent organisation, and a passion for maintaining high standards of care.

Key Responsibilities include:

  • Supporting the Registered Manager in daily operations.

  • Preparing rotas in advance and ensuring safe staff allocation.

  • Overseeing safeguarding concerns and ensuring best practice.

  • Line managing the Head of Care and Care Coordinators.

  • Managing annual leave within the service.

  • Assisting Care Coordinators with contingency planning.

  • Covering staff sickness in emergencies.

  • Acting as a key point of contact for both staff and service users.

  • Conducting care plan assessments, reviews, and staff supervisions.

  • Representing the service in multi-disciplinary team (MDT) meetings.

  • Working independently on service initiatives with minimal direction.

This is a hands-on role that requires strong communication, problem-solving, and leadership skills, ensuring the service runs smoothly and continues to deliver safe, person-centred care.

This is a temporary position on a 12 month contract, with other job opportunities within the company after the 1 year.

Care Co-Ordinator

We are seeking an enthusiastic and motivated Care Co-ordinator to join our management team. This is a pivotal role where you will support both our service users and care staff, ensuring the smooth running of daily operations and maintaining the highest standards of care. You will be responsible for rota management, staff supervision, care planning, and being a key point of contact for both colleagues and external professionals. This is an excellent opportunity for someone with strong leadership, organisation, and communication skills who is passionate about delivering person-centred care.

Key Responsibilities include:

  • Manage daily rota changes and communicate with Community Carers.

  • Monitor and manage the Digital Care system, following up on alerts and tasks.

  • Provide hands-on care when necessary to cover holidays and sickness.

  • Work flexibly between 07:00–22:00 on designated working days.

  • Share responsibility for on-call duties with the other Care Co-ordinator, providing seven days of cover.

  • Supervise staff, including work-based assessments, reviews, and spot checks.

  • Prepare and review person-centred care plans.

  • Support care staff on shift and ensure services run safely.

  • Act promptly on safeguarding concerns.

  • Liaise with multi-disciplinary professionals for the wellbeing of service users.

  • Ensure confidentiality and regulatory compliance at all times.

  • Manage and support service users on re-ablement programmes.

Role Requirements:

  • Minimum NVQ Level 3 in Health & Social Care (or actively working towards).

  • Foundation level Maths and English.

  • Strong communication and interpersonal skills.

  • Proficient in IT and digital care systems.

  • Ability to work independently and as part of a team.

  • Adaptable, with strong leadership and problem-solving skills.

  • Knowledge of care planning and audit processes.
